* 1. Please complete the following and pay particular attention to the loan period for which you request the loan. If you choose Fall 2006, Spring 2007, Summer 2007 as your loan period, your loan will be broken up into 3 disbursements and you will not receive the 3rd disbursement until the summer semester.

* 4. Preferred Lender List: Please place a check mark beside your Lender selection. If you would like to borrow from a lender not list below, select the OTHER option and provide the requested information. Please note that if you are a first time borrower or select a different lender for the 2007-08 academic year, you will be required to complete a new Master Promissory Note (MPN).

5. A “Current Borrower Benefits” chart, comparing the benefits offered by each of the lenders listed below, is available upon request from the Student Financial Aid Office. You can also review the benefits offered by lenders at the lenders’ student loan web sites.

I understand that it is my responsibility to return the Master Promissory Note to my chosen lender/servicer.

I understand that I am required to complete both an Entrance Interview Session and Exit Interview Session and turn in the required paperwork to the Financial Aid Office annually. I also understand that I must attend an Exit Interview Session or complete the Exit Interview Session on-line at any time I leave school whether by graduating, transferring or withdrawing.

I understand that my grades, transcripts, and diploma (if I am graduating) will be held up if I do not comply with the above policies.
