Consent Statement

According to the Family Educational Rights and Privacy Act (FERPA) we can not share a students information with anyone but the student (this includes the students parents).  To speak with anyone but the student we need the consent of the student. 

This form allows the Financial Aid Office to speak to or share information with whoever you want.  Without this form we cannot share information with anyone but you. 

You can also give permission to other offices at Hagerstown Community College (i.e. Records and Registration) to speak to this person.

Print the form; fill out completely and return to the Financial Aid Office.
