TRiO SSS - Letters of Recommendation

If you would like to request a letter of recommendation from a TRiO SSS staff member, please complete the following form no later than two weeks before the letter of recommendation is due.

Change of Major Form

The Change of Major Form is used when a student needs to change their current active academic major (program of study), or update their academic major to the new catalog year for degree requirements, and/or add an additional major or credential. Students need to meet with an Academic Advisor to review and complete the Change of Major Form. To meet financial aid requirements it is critical that students be in the correct academic major for aid to be awarded.

2018-2019 Foster Care Tuition Waiver Form

Fill out this form if you are, or have been in the past, a foster care youth or are directed to fill it out by the Financial Aid Office. You must be under the age of 25 to be eligible for the Tuition Waiver.

You cannot submit this form online. Please fill out all fields, sign and return to the Student Financial Aid Office.

2018-2019 Homelessness Form

Fill out this form if you, the student, reported that you are homeless on the FAFSA, and the Student Financial Aid Office is requesting written documentation to determine that you are homeless. Submit this form ONLY if it is requested by the Financial Aid Office.

2018-2019 Unusual Enrollment History Appeal Form

The U.S. Department of Education has established regulations to prevent fraud and abuse in the Federal Pell Grant Program by identifying students with unusual enrollment histories. Some students who have an unusual enrollment history have legitimate reasons for their enrollment at multiple institutions. However, such an enrollment history requires a review to determine whether there are valid reasons for the unusual enrollment history. This must be resolved before you will receive financial aid.
