Unusual Enrollment History Appeal

The U.S. Department of Education has established new regulations to prevent fraud and abuse in the Federal Pell Grant Program by identifying students with unusual enrollment histories. Some students who have an unusual enrollment history have legitimate reasons for their enrollment at multiple institutions. However, such an enrollment history requires a review to determine whether there are valid reasons for the unusual enrollment history. This must be resolved before you will receive financial aid.

Stafford Loan Prior Default Appeal Form

A Student who has had Federal Stafford Loans in default status may need to give additional documentation before receiving Stafford Loans again- submit this form if you are requesting Federal Stafford Loans for the academic year.

Important: You are not eligible for any Financial Aid if any Federal Loans are currently in default status.

Selective Service Form

If you are a male, between the ages of 18 and 25, you must register for Selective Service to receive Federal Financial Aid.  If you did not register through the FAFSA, please go to the Selective Service website and register.  Please submit a copy of your Selective Service Registration Card to the Financial Aid Office.

If you are a male that is under 18 and will not have your 18th birthday until after the school year begins, please use this form and fill out the appropriate section.

Satisfactory Academic Progress Appeal Form

Use this from if you have received a letter from Financial Aid that you are no longer eligible for Financial Aid due to your GPA being low or withdrawing from classes.

You can appeal this decision by filling out this form, following the directions and submitting it to the Financial Aid office.
