2019-2020 Income Reduction Form

If your income has recently decreased or you have special financial issues that were not taken into account on your FAFSA, we may be able to reevaluate your financial and/or income information using the 2018 tax year or another 12 month time period that better reflects your financial circumstances. Print this form and follow the directions on the first pages, then submit to the Financial Aid Office.

2019-2020 Unusual Enrollment History Appeal Form

The U.S. Department of Education has established regulations to prevent fraud and abuse in the Federal Pell Grant Program by identifying students with unusual enrollment histories. Some students who have an unusual enrollment history have legitimate reasons for their enrollment at multiple institutions. However, such an enrollment history requires a review to determine whether there are valid reasons for the unusual enrollment history. This must be resolved before you will receive financial aid.
