A Student who has had Federal Stafford Loans in default status may need to give additional documentation before receiving Stafford Loans again- submit this form if you are requesting Federal Stafford Loans for the academic year.

Important: You are not eligible for any Financial Aid if any Federal Loans are currently in default status.
