Your application was selected for review in a process called “Verification.” In this process, the Financial Aid Office will be comparing information from your application with the information that you fill out on this form.
In the past, Hagerstown Community College has needed both signed Federal Tax Returns and W2s from students (spouses) and parents of dependent students who were selected for Verification. However for 2012-2013, students (spouses) and parents of dependent students will need to submit an IRS Tax Transcript if requested, which must be obtained from an Internal Revenue Service Office, not your tax preparer. Check "My Financial Aid Documents” under the Student Communication tab on Web Advisor to determine if you need to submit Tax Transcripts with this form.

Please note: Importing your tax info with IRS data retrieval tool on the FAFSA may save you the trip to the IRS office.
The law says we have the right to ask you for this information before awarding Federal aid. If there are differences between your application information and your financial documents, the Financial Aid Office may need to make corrections electronically.
Submit this form ONLY if it is requested on WebAdvisor under "My Financial Aid Documents", or requested to accompany another form.
