If you (or your parent) filed an automatic tax extension for the 2012 tax year and are unable to provide IRS tax return transcripts, please complete and submit this form with all applicable documentation as listed on the form. It is highly recommended that you submit a copy of your IRS Tax Return Transcript (the official IRS print-off of your processed tax return) about three weeks after you file.
The law says we have the right to ask you for this information before awarding Federal or State aid. If there are differences between your application information and your financial documents, the Financial Aid Office will need to make corrections electronically.
