Annual Professional Development Plan Forms for Faculty
   
Purpose

To be used by faculty to write an annual professional development plan in consultation with the faculty member’s Division Chair / Director.

Procedure

  1. The plan must be completed annually for the new fiscal and academic year not later than May 30, and submitted electronically to the division chair or director.
     
  2. In section one fill in the name, rank, tenure, years of service, department, and division.
     
  3. In section two enter goals and outcomes. Goals should reflect a faculty member’s desires and necessity for professional development. Goals may involve activities such as continuing education, graduate courses, certification, developing a new course or program, pursuing grants, or working with students, to name just a few. A faculty member’s goals should be congruent with department goals and the college’s strategic plan.

    At least one goal should include a faculty member’s involvement with college or department activities or projects associated with student learning outcomes. A second goal should include a faculty member’s involvement with college or community service.

    Outcomes must be established for each goal. Goals may have more than one outcome. Outcomes must be measurable.
     
  4. In the “faculty duty” section list the faculty duties that each goal represents. A number representing the specific faculty duty can be entered or the specific faculty duty written out.
     
  5. In the last section identify whether the goal is a short term or a long term one. A short term goal is one that can be accomplished by the end of the academic year. A long term goal is one that takes longer than one academic year to complete. When feasible, enter specific dates for completion.
     
  6. In section three list established goals in column one by number. In the space next to each number complete a short narrative that explains the goal and its relationship to the department’s goals and the college’s strategic plan.
     
  7. In section four the faculty member signs the plan and submits it to the Division Chair/Director for discussion and approval. When approved, the Division Chair/Director signs the form and submits it to the Dean of Academic affairs for review and approval. Once signed, the form is forwarded to the President for a final review and approval.
     
  8. Throughout the year, the faculty member should review the professional development plan. In section five, information that shows that progress is being made in meeting the goal can be entered. Enter the goal number and the date, and then a short explanation. Division chairs may require a meeting in January or February to discuss a faculty member’s progress. Division chairs will schedule a formal meeting in January for February for faculty who are applying for promotion or tenure.

    As a goal is completed enter the date and a short explanation of whether all of the outcomes were achieved.

    Goals listed at the end of the year as partially met, must include a short explanation of which outcomes were met, and those that were not, and why.

    Goals listed at the end of the year as not met, must include an explanation of why the goal was unable to be accomplished.
     
  9. In section six, faculty can enter a short explanation of any additional accomplishments they have achieved over the past year.
     
  10. All Division Chairs/Directors will meet individually with division faculty by May 30 to conduct a formal review of a faculty member’s professional accomplishments and performance during the academic year. A short summary, or specific comments, can be written in section six by the Division Chair/Director. This may include topics related to the faculty member’s goals and accomplishments, student evaluations, classroom observations, or the faculty member’s general performance as a team member in the division, to name just a few.
     
  11. After the individual meeting is completed, both the faculty member and the Division Chair/Director must sign and date the form. A copy is given to the faculty member and the original placed in the faculty member’s employee file.
