Unusual Enrollment History Appeal

The U.S. Department of Education has established new regulations to prevent fraud and abuse in the Federal Pell Grant Program by identifying students with unusual enrollment histories. Some students who have an unusual enrollment history have legitimate reasons for their enrollment at multiple institutions. However, such an enrollment history requires a review to determine whether there are valid reasons for the unusual enrollment history. This must be resolved before you will receive financial aid.

Stafford Loan Prior Default Appeal Form

A Student who has had Federal Stafford Loans in default status may need to give additional documentation before receiving Stafford Loans again- submit this form if you are requesting Federal Stafford Loans for the academic year.

Important: You are not eligible for any Financial Aid if any Federal Loans are currently in default status.

Selective Service Form

If you are a male, between the ages of 18 and 25, you must register for Selective Service to receive Federal Financial Aid.  If you did not register through the FAFSA, please go to the Selective Service website and register.  Please submit a copy of your Selective Service Registration Card to the Financial Aid Office.

If you are a male that is under 18 and will not have your 18th birthday until after the school year begins, please use this form and fill out the appropriate section.

Required Housing Status Form

If the Financial Aid Office has requested this form, you indicated on your FAFSA that you will live on campus at HCC. We do not have on campus housing. Print this form and make a choice on the form as to where you will live while you are attending HCC, and return to our office.

Submit this form ONLY if it is requested on WebAdvisor under "My Financial Aid Documents."

Iraq and Afghanistan Service Grant Verification Form

The Financial Aid Office will request this form if you have indicated that you may be eligible for the Iraq and Afghanistan Service Grant.

A student who is not eligible for a Pell Grant but whose parent was a member of the U.S. Armed Forces and died as a result of service performed in Iraq or Afghanistan after September 11, 2001 may be eligible to receive the Iraq and Afghanistan Service Grant.

If you do not have an e-mail address at this time, or you do not provide us with an alternative address, the Financial Aid Office will use the e-mail assigned by Hagerstown Community College or the e-mail address you listed on your FAFSA.

Determination of Homelessness Form

Fill out this form if you, the student, reported that you are homeless on the FAFSA (Free Application for Federal Student Aid), and the Student Financial Aid Office is requesting written documentation to determine that you are homeless.

Consortium Agreement

You will need this form if you are receiving Financial Aid at HCC, and are attending another college or university at the same time.

You can attend two colleges at the same time, BUT you cannot receive Financial Aid at two different colleges or universities at the same time. Please talk to someone in Financial Aid Office before using this form.
