Selective Service Form

If you are a male, between the ages of 18 and 25, you must register for Selective Service to receive Federal Financial Aid. If you did not register through the FAFSA, please go to the Selective Service website and register. Please submit a copy of your Selective Service Registration Card to the Financial Aid Office.

If you are a male who is under 18 and will not have your 18th birthday until after the school year begins, please use this form and fill out the appropriate section.

Housing Status Form

You indicated on your FAFSA that you will live on campus at HCC. We do not have on campus housing.

Iraq and Afghanistan Service Grant Form

A student who is not eligible for a Pell Grant but whose parent was a member of the U.S. Armed Forces and died as a result of service performed in Iraq or Afghanistan after September 11, 2001 may be eligible to receive the Iraq and Afghanistan Service Grant.

Additional Student Eligibility Requirements:

  • Be under 24 years old or
  • Enrolled in college at least part-time at the time of the parent’s or guardian’s death.

Consortium Agreement

You will need this form if you are receiving Financial Aid at HCC and are attending another college or university at the same time. You can attend two colleges at the same time, BUT you cannot receive Financial Aid at two different colleges or universities at the same time. There is a deadline to submit this form, so please talk to someone in Financial Aid Office before using this form.

Bachelor's Degree Form

This form is requested because you indicated on your FAFSA that you have already received your Bachelor (or 4 year) degree or that you would be pursuing a graduate degree.

Parent Loan for Undergraduate Students (PLUS)

Parent loans are designed for parents who want to borrow to help pay for their dependent children’s educational expenses. Parent borrowers generally must begin repaying the principal and interest within 60 days after the loan is completely disbursed.

Students and parents wishing to apply for education loans must meet all requirements for federal financial aid, and parents must complete a Master Promissory Note on the StudentLoans.gov website.

Excessive Debt Appeal Form

Students should consider carefully how much they will need to borrow and take into account the potential future burden of repaying loans after college. Students who are planning to transfer to a four-year college are advised not to borrow additional loans at the community college level if grants, scholarships, or other payment arrangements are available.
