Supplemental Application for the Dental Hygiene Program

BEFORE SUBMITTING A SUPPLEMENTAL APPLICATION FOR A HEALTH SCIENCE PROGRAM, YOU MUST SUBMIT AN ADMISSION APPLICATION.

This supplemental application, official transcripts from all colleges attended (HCC calculates cumulative GPA from all college coursework), the Dental Hygiene Observation form and official scores for the ACT® WorkKeys® National Career Readiness Assessment must be received by the April 1 deadline to be eligible for the program. The following credits must be completed or in progress at the time of the application to be eligible for admission. The courses include ENG 101, BIO 116 (or BIO 203 must be completed and BIO 204 must be in progress), CHM 101, MAT 109 (or an approved General Education Math course), and BIO 205. Recommended courses completed prior to application are SPD 108, a 3-credit Arts and Humanities course from approved General Education course list, and PSY 101. Additional information can be accessed at the Registration office or in the HCC Catalog. Each student must submit an HCC Dental Hygiene Observation Form documenting 8 hours of observations. Observations older than 3 years will not be accepted. Each student must submit official scores for the ACT® WorkKeys® National Career Readiness Assessments. The assessments measure foundational skills required for entry in the dental hygiene program. Applicants need to achieve a WorkKeys® assessment level of 5 in Applied Math, 5 in Graphic Literacy, and 4 in Workplace Documents taken within the last three years. Program acceptance or denial letters will be mailed out by mid-May. Please note that all accepted students must attend the mandatory orientation which is usually held the second Friday in June.
