Faculty Reporting Forms

Academic Integrity Reporting Forms (Faculty Use Only)

For reporting academic dishonesty. Submit as part of Step 1 in the Academic Integrity Violation Process. Reports go to the Vice President for Academic Affairs & Student Services.

  • Academic Integrity Warning (AIW):
    Educational warning for minor or first-time offenses. Not a conduct violation, but recorded as a warning. Repeated issues may lead to further action.
  • Academic Integrity Violation (AIV):
    Formal report with possible course failure. Recorded as a conduct violation on the student’s disciplinary record. Repeat violations may result in additional consequences.
