Please use the appropriate form below to report student concerns or issues.

Student Assistance Report (CARE)

Use this form to report students facing personal challenges that may impact academic success or course registration decisions. Students may also self-submit for support. A CARE team member will follow up to offer help.

Student of Concern Report (BIT)

Submit this form for students exhibiting concerning, unusual, or potentially threatening behavior—including signs of self-harm or aggression. This form is reviewed by the Behavioral Intervention Team (BIT).

Behavioral Conduct Report (Code of Student Conduct)

Report behavior that may violate HCC’s Code of Student Conduct. If the behavior does not meet this threshold, use the Student of Concern Report instead. This form initiates the formal conduct process.

Faculty Reporting Forms

Academic Integrity Reporting Forms (Faculty Use Only)

For reporting academic dishonesty. Submit as part of Step 1 in the Academic Integrity Violation Process. Reports go to the Vice President for Academic Affairs & Student Services.

  • Academic Integrity Warning (AIW):
    Educational warning for minor or first-time offenses. Not a conduct violation, but recorded as a warning. Repeated issues may lead to further action.
  • Academic Integrity Violation (AIV):
    Formal report with possible course failure. Recorded as a conduct violation on the student’s disciplinary record. Repeat violations may result in additional consequences.
