Consortium Agreement

You will need this form if you are receiving Financial Aid at HCC and are attending another college or university at the same time. You can attend two colleges at the same time, BUT you cannot receive Financial Aid at two different colleges or universities at the same time. There is a deadline to submit this form, so please speak with a staff member at the Student Financial Aid Office before using this form.

You cannot submit this form online.

Please fill out all fields, sign and return to the Student Financial Aid Office. You may scan and email, fax, mail or submit forms in person. Our address, fax number and email address are provided on the form.
